Take action to prevent diabetes
Diabetes is a common, serious health condition that causes a higher than normal level of glucose in the blood. Most glucose found in the body comes from the foods we eat. During a normal digestive process, as the body breaks down food, sugar enters the bloodstream. The pancreas reacts by secreting a hormone called insulin to move the sugar through the bloodstream, allowing sugar to enter cells and reduce the amount of sugar present in the bloodstream. Glucose levels become elevated when the body either does not produce enough insulin or the body is unable to use insulin properly. The glucose remains in the blood rather than being used by cells for energy. Additionally, individuals with diabetes are at an increased risk for developing other health issues including glaucoma, neuropathy, heart disease, kidney disease, and stroke.
There are several different forms of diabetes. The most common include Type 1, Type 2, Gestational and pre-diabetes. Type 1 diabetes, formerly referred to as juvenile diabetes, is most commonly diagnosed in children and young adults and occurs when the pancreas doesn’t produce enough insulin. Type 2 diabetes is more commonly diagnosed in adults and is a result of the pancreas making less insulin as well as the body’s insulin not working efficiently. Gestational diabetes occurs in women whose blood sugar becomes elevated during pregnancy. Type 1, Type 2, and Gestational diabetes can all be controlled.
Normal fasting blood glucose levels fall between 70-99 mg/dL. A fasting blood glucose level of 126 mg/dL or higher is in the diabetes range. When the fasting blood sugar levels are higher than the target range but not high enough to be in the diabetes range, the individual has pre-diabetes (fasting glucose between 100 to 125 mg/dL). In general, people with pre-diabetes have no symptoms. This is why it is so important to keep up with annual wellness exams and routine blood work. You may notice some of the earlier signs of diabetes including: increased thirst, hunger or urination, blurry vision, or increased fatigue.
There are some factors that increase a person’s risk for developing pre-diabetes. Those factors include:
Weight – Being overweight increases the risk for pre-diabetes. Excess fat can lead to insulin resistance so that the body’s insulin doesn’t work properly. Keeping your BMI below 25 (below 23 for Asian Americans) will decrease your risk.
Waist Size – The risk for insulin resistance increases in men with waists larger than 40 inches and women with waists larger than 35 inches.
Diet - Dietary factors like eating red or processed meat and drinking sugary beverages increases your risk while diets rich in fruits, vegetables, nuts and whole grains lower your risk.
Inactivity - The less active you are, the higher your risk for developing pre-diabetes. Staying active helps you maintain a healthy weight, burn glucose and make cells more sensitive to insulin.
Age - Diabetes and pre-diabetes can develop at any age, but the risk is higher after the age of 45.
Family History - Pre-diabetes is more likely if a parent or sibling has type 2 diabetes.
Race - Although it is unclear why, African-Americans, Hispanics, Native Americans, Asian-Americans and Pacific Islanders have increased risk of developing pre-diabetes.
Gestational Diabetes - Women who develop gestational diabetes while pregnant have an increased risk of developing pre-diabetes later in life.
Polycystic Ovary Syndrome - This common female condition is linked to higher instances of pre-diabetes.
Sleep - Individuals with sleep disorders, including sleep apnea, or who work night or rotating shifts are more likely to develop insulin resistance.
Other health issues linked to the development of pre-diabetes include:
- High blood pressure
- Low HDL (“good” cholesterol)
- High triglyceride levels (fat found in blood)
Pre-diabetes should be treated as a warning sign from your body that lifestyle changes are needed to prevent type 2 diabetes. Uncontrolled type 2 diabetes can lead to the development of several other health issues such as heart disease, kidney issues, blindness or stroke. It is possible to reduce your risk of developing type 2 diabetes by eating a well-balanced diet, engaging in physical activity and maintaining a healthy weight.
Consult with your primary care physician if you have a family history of diabetes, or if you notice any symptoms of diabetes. A blood glucose test should be performed a minimum of every three years after the age of 45. Your physician may want to begin monitoring earlier and more frequently if you have a family history or are at an increased risk of developing pre-diabetes.

Outpatient progress notes are available for review in the 'Past Appointments' section of MyChart. Simply click on a past appointment to review the progress notes written by your Cleveland Clinic provider for the visit.
Medications
You can view a list of your current medications, request prescription renewals and learn more about your medications by clicking on 'About This Medication.'
Preventive Care Information

The 'Download Your Health Summary' feature enables you to download PDF copies of your after-visit summary and past medical history to share with other providers or use for your own personal benefit.
Provide Information Access to Referring Doctors
MyChart users can authorize a non-Cleveland Clinic doctor to access their medical information by selecting 'DrConnect Authorization' in MyChart under 'Manage My Health Info'.
If you approve release of your medical records to your doctor, he or she will need to be registered through DrConnect for access. We will verify that your doctor is registered or contact him or her to sign up.

Complete an eVisit for Minor Health Issues
eVisits are a convenient, secure way to access Cleveland Clinic providers for seven common, minor ailments:
- Adult pink eye/red eye
- Back pain
- Coughs
- Diarrhea
- Sinus issues
- Urinary tract infections
- Vaginal yeast infections
Complete a questionnaire regarding your symptom history and a provider will respond with a diagnosis and treatment instructions within one business day. If medication is prescribed, the prescription order will be sent directly to your preferred pharmacy.
Message Your Providers
The Message My Doctor’s Office feature enables you to send a secure message regarding a non-urgent medical concern to your established provider’s office and receive a response from the healthcare team within three business days. An established provider is one you have seen within the last fifteen months.
Schedule and Request Appointments
MyChart enables you to view your established provider’s open schedule and book an appointment online in real-time. This feature is available in internal medicine, family medicine, geriatrics, or pediatrics at any of our family health center locations, and internal medicine at main campus.
MyChart also enables you to request appointments for your established providers in any department.

What are eVisits?
Cleveland Clinic MyChart eVisits are an easy way to access care for non-urgent issues from the comfort of home or while on-the-go, at any time that is convenient for you. Available through Cleveland Clinic MyChart website or mobile app, this feature enables you to quickly submit information regarding your health, and a caregiver can diagnose and prescribe if appropriate, without needing an appointment.
When should I use an eVisit?
An eVisit can only be used for non-urgent symptoms related to these minor illnesses:
Adults (ages 18+)
- Adult pink eye/red eye
- Back pain
- Coughs
- Diarrhea
- Sinus issues
- Urinary tract infections
- Vaginal yeast infections
Pediatrics (ages 2-17)
- Conjunctivitis (pink eye)
- Lice
- Upper respiratory infection (cold)
Who can submit an eVisit?
eVisits are available to patients age 2 and over who are currently located in the state of Ohio and have had a visit with a Cleveland Clinic provider in the last three years.

An eVisit for a common, minor health concern costs $20 and is not currently covered by insurance. The fee will be billed to you after the eVisit is complete.

How do I sign up for MyChart?
There are two convenient ways to establish your personal MyChart account:- The next time you visit your Cleveland Clinic provider, ask your patient service representative at the front desk to generate a one time-use Activation Code for your personal MyChart account.
- Click the “Sign Up For MyChart” button above, and complete the online MyChart sign-up form by entering the requested information. (Please note that the information you provide is confidential and it will be processed through our secure website). Your information will be verified; then a special, one-time-use MyChart Activation Code will be sent to you via e-mail. Once you receive your one-time-use Activation Code, please use it to activate your MyChart Account.
How can I get my Medical Record Number?
Your Cleveland Clinic Medical Record Number can be found on an appointment reminder or an after visit summary.
Can I activate MyChart for a loved one?
Yes you can. MyChart users that are parents or legal guardians of minor children (age 17 years and younger) can add their loved ones to their personal MyChart account to access portions of their child's medical records.
MyChart can also be used by caregivers with court-appointed guardianship of dependent adults to access portions of the medical record of their loved ones.
In addition, adults can provide legal authorization that will enable their family members or caregivers to manage their healthcare online through MyChart.
You will have the same convenient, 24-hour online access to medical information and health management services for your dependent loved one as you enjoy with your own MyChart account.

When I can see my recent test results?
Most test results are automatically released to MyChart. It may take several days or even weeks for certain tests to be completed and reported. If you feel that your test results should be available but you don't see them in MyChart, it is best to call the office of the provider who ordered the test(s).
